Referral Form

We are now accepting referral letters for children with life-threatening, critical illnesses and severe injuries so that we can make their dreams come true. The referral can come from a parent, care-giver, close family members, or medical professional... anyone with knowledge of the child's situation. We would even accept letters from referral children in need themselves. Children between the ages of 2 1/2 and 17 years of age and living in Southeastern Minnesota may be referred. All referrals will be evaluated for meeting eligibility requirements. Please suggest two or three potential dreams keeping in mind that we are a new organization and are still building our funds.  

Counties in SE Minnesota that we are currently accepting referrals from include: Blue Earth, Dodge, Faribault Fillmore, Freeborn, Goodhue, Houston, Le Sueur, Mower, Olmsted, Rice, Steele, Wabasha, Waseca, and Winona

Parents/Guardians of recipients under consideration will be contacted for further information.
